Ottawa SPIN Survey/Registration Form

Dear Colleague: This survey is intended to do three things:
  1. Register you as a member of the Ottawa Software Process Improvement Network (Ottawa SPIN)
  2. Gather some information on the types of topics and activities that interest the SPIN audience
  3. Solicit help in launching and running the Ottawa SPIN (first meeting tentatively June 13!).

The membership information will be compiled and put into a directory/mailing list that will be made available to members. Also, the data derived from this survey will be used to plan future meetings. Please fill out the information below. If there are other people in your organization or in other organizations you know who would be interested in attending or helping, please have them fill out this form, too, so that we can estimate our audience and our support.

The following is intended to get some rough ideas on future SPIN meeting topics/activities. Please check those ideas that would best suit your needs. Add any additional comments in the spaces provided.
